A.1.1. ESTADIO I

Los pacientes con enfermedad estadio IA no se benefician de quimioterapia (QT) complementaria. En el estadio IB hay resultados dispares en los estudios aleatorizados y no es recomendada de modo estándar, aunque podría valorarse de forma individual en los pacientes considerados de riesgo: pobremente diferenciados, invasión vascular, mayores de 4 cms, afectación de pleura visceral.

A.1.2. ESTADIOS II-III

Los datos de los ensayos clínicos randomizados publicados en los últimos 10 años demuestran beneficio en supervivencia en los pacientes resecados que han recibido QT adyuvante con esquemas basados en platino, considerándose una indicación estándar. Los ensayos IALT, JBR10 y ANITA muestran una reducción del 14 al 31% del riego de muerte a 5 años tras la administración de 4 ciclos de QT adyuvante.

Dos meta-análisis posteriores confirman este beneficio. Por ejemplo: el Lung Adjuvant Cisplatin Evaluation (LACE) analiza los datos de 4.584 pacientes incluidos en 5 estudios publicados (ALPI, ANITA, BLT, IALT y JBR10) en los que el brazo de tratamiento incluía esquemas basados en cisplatino. El beneficio absoluto de la QT en supervivencia fue del 5.3% a los 5 años. Este beneficio se mantiene en población anciana ó mayor de 65 años, sino presentan comorbilidades asociadas.
